If I want to claim our medical expenses that we paid out of pocket, do I include the mileage in that?

Yes, you may include the mileage associated with medical expenses. Please note that the rate for 2014 is 23.5 cents. Thus, if you had driven 500 miles, you would add 500 x 23.5 cents or $117.50 to the medical expense line 1 on Schedule A.
